We must have your 2008-09 Hillside Student Information/Medical form on file

To reserve your spot you must turn in...

Hume Lake Medical Release Form (white form, BOTH SIDES need to be signed)
Hillside Permission slip (white 1/2 sheet)
$100.00 deposit (Non-Refundable, Non-Transferable Deposit)

The full amount of $425 (this includes your deposit) is due no later than Sunday, June 22. The increase in price is due to Hume Lake raising the cost for the week.

Meet at Hillside Church at 8:30am on Sunday, July 13
We will return on Saturday, July 19 at 3pm
